Please read this declaration carefully before you submit this form.
By completing the following declaration you are confirming that the business making the application is eligible for this grant payment and has not exceeded the following 3 subsidy allowances:
•Small Amounts of Financial Assistance Allowance – you’re allowed up to £335,000 (subject to exchange rates) over any period of 3 years.
•COVID-19 Business Grant Allowance – you’re allowed up to £1,600,000
•COVID-19 Business Grant Special Allowance - if you have reached your limits under the Small Amounts of Financial Assistance Allowance and COVID-19 Business Grant Allowance, you may be able to access a further allowance of funding under these scheme rules of up to £9,000,000, provided certain conditions are met.
Grants under these 3 allowances can be combined for a potential total allowance of up to £10,935,000 (subject to exchange rates).
It is the responsibility of the applicant to ensure that your business does not exceed these allowances by receiving this grant funding and by completing the declaration you accept that any grant funding that is unlawfully claimed or paid in error will be subject to clawback from your business.
In addition, I declare that as at 1st April 2021 that the applicant's undertaking was not in the financial difficulties. (For the avoidance of doubt, businesses that were in administration, are insolvent or where a striking-off notice has been made at the date of the local lockdown are not eligible for funding under this scheme).
I understand that knowingly providing incorrect information, or knowingly making a false declaration, could lead to a prosecution under the Fraud Act 2006. I claim the grant, and declare that I am authorised to act on behalf of the above named account holder and are entitled to the grant.
The grant payment is taxable and I acknowledge that this will need to be included as income in my tax return.
