CCTV Request Form - Sch2 Pt1 (5)


Please read before completing application

Important – this form is relevant if you have: 
  • Been the victim of a crime 
  • Been involved in a road traffic collision or
  • Are an insurance company or legal representative
Before completing this application form, please refer to the guidance on this page and the information provided on the Peterborough City Council CCTV information page (www.peterborough.gov.uk/cctv). 

Please be advised of the following: 

If you have been the victim of a crime, or it is a crime-related incident, you should report the matter direct to the Police. The Police can make a request direct to the Police Liaison Officer and, if held, footage will be provided directly to the investigating officer. 

If you have been involved in a road traffic collision and the Police are investigating, the Collision Unit can make a request direct to the Police Liaison Officer as detailed above. 

If you are an insurance company or legal representative you may request footage under Schedule 2 Part 1 (5) of the Data Protection Act 2018. There is a charge of £100 + VAT which is only payable online to Peterborough City Council. 

CCTV footage is held by the council for a period of 30 days only. Any footage requested beyond this time limit will not be available.
Please ensure you complete ALL sections of the application form, as incomplete forms may cause a delay with your application. 

Section 1: Asks you to provide details of your organisation, contact details and reference number. 

Section 2: Asks you to provide details of the precise location, date and time of the incident you require.

Assistance for completing below fields

Precise Location


In order to assist a search for footage, the precise location should be provided. It is not sufficient to simply state ‘High Street Peterborough’.


A location could be as below:


  • High Street Peterborough, at its junction with Station Road
  • High Street Peterborough, 20 yards North / Lincoln Side of its junction with Station Road.
  • These locations could be supplemented with a secondary location i.e. outside smiths Fish and Chip shop. This should be a secondary location and not a primary location.
  • In the case of a dual carriageway, which carriageway was being used, ie Lincoln Bound carriageway.


Description of Incident


Remember to include make, model, colour and registration numbers of all vehicles involved. The more information you are able to provide, will assist in securing footage if available.


For example:


It is alleged that Red Ford Escort index ABC123 turned right from High Street into Station Rd across path of White Datsun Sunny index XYZ456 and a collision occurred.



Description of person/s relevant to the application


Details should include:


  • Gender
  • Hair – state whether short, long, colour, bald, shaven head
  • Clothing – for example: coat, jumper, shirt, blouse, t- shirt, hoodie, hat. Remember to include colour of clothing and any distinctive markings such as logos
  • Further factors – state if carrying a bag, holdall, rucksack, handbag, shopping bags. Remember to include colour and any distinctive markings such as logos

(A period of one hour only will be supplied. If no end time given footage will cover 30 mins before and 30 mins after.)

Section 3: Supporting documents

It is import that you provide supporting documentation with your application. 

This is required to ensure, in respect of insurance companies or legal representatives, that you are authorised to act on behalf of your client. Photographic evidence will also assist us in identifying your client in any footage. If you have a copy of the V5 Registration document for your vehicle, you may also wish to provide this to assist with your application.

Reminder: the fee is payable even if the incident has not been captured.
